INFORMATION FROM YOUR SCHOOL NURSE REGARDING IMMUNIZATIONS AND MEDICATIONS


Kindergarten Parents:

There will be an Immunization Clinic set up by the Texas Department of State Health during Kindergarten Registration at Brock Elementary School Cafeteria on May 7th from 10-1pm. Students should take advantage of this opportunity to get their immunizations up to date. Cost of immunizations is on a sliding scale according to ability to pay.

Please bring immunizations records and Medicaid card if available.

Important: There will be no grace period for vaccines in the 2008-09 school years. Current vaccine records will be required for the student to attend the first day of class. There will be no exceptions!!

If your child is taking any medications at school, please complete the Medications Administration Form.

Freshman Parents:

Please review your child’s vaccination records or check with your primary care provider concerning the Tetanus Booster. Around the age of fourteen, most school age children are required to get another Tetanus vaccine. The State requires one vaccine every 10 years. This vaccine is required by the State for Public School Attendance. Please verify that your child is in compliance so that hopefully attendance does not become an issue.
Your child may attend the Immunization Clinic on Wednesday May 7th from 9am to 1:00 pm at the elementary school cafeteria if a tetanus booster is needed. The cost is on a sliding scale according to ability to pay. A consent form will need to be signed if parent unable to be present.

All Parents:

DELEGATION OF AUTHORITY TO GIVE INFORMED CONSENT FOR IMMUNIZATIONS OF A MINOR
